Uber Tristram (Pandemonium Event)

Collect the three keys in Hell: Key of Terror (The Countess, Act 1), Key of Hate (The Summoner, Act 2), Key of Destruction (Nihlathak, Act 5).
Put the 3 keys in the Horadric Cube to open a portal to one of Matron's Den, Forgotten Sands or Furnace of Pain (which one opens is random).
So you need 3 sets of 3 keys: open all three dungeons, kill the bosses and collect the organs. Lilith -> Diablo's Horn, Duriel -> Baal's Eye, Izual -> Mephisto's Brain.
Put Diablo's Horn + Baal's Eye + Mephisto's Brain in the Cube to open the Tristram portal.
Killing Uber Mephisto, Uber Diablo and Uber Baal (the Prime Evils) drops an unidentified Hellfire Torch.

The Standard of Heroes that drops alongside it has no effect, so most players just throw it away. The Torch must be identified to determine which class its +3 class skills belong to.
